If Your Hormone Pellet Provider Is No Longer Available: A Patient's Guide to Continuity
Marie Perkowski, CRNP, FNP-BC ·
I wrote this for anyone with questions about hormone pellet therapy — whether you're new to it, considering it, or have been on it for years. The goal is to make the information easy, honest, and in your hands.

Here's what you actually need to know.
Why pellet continuity matters
Pellet therapy isn't like refilling a prescription. Your pellets release hormone gradually over 3-6 months. The dose you received last time was individualized based on your labs, your symptoms, and your body's response. A new provider can't just repeat what you had — they have to understand your trajectory to pick up where you left off safely.
Gaps in pellet dosing matter. Hormone levels decline predictably once pellets wear off. Depending on how long you've been without a new insertion, you may feel it: fatigue, mood shifts, hot flashes returning, sleep disruption, loss of energy and muscle tone. The longer the gap, the more pronounced the adjustment when you eventually re-dose — which is one reason you want to find continuity care before you cross that threshold.
What to gather before your first visit
Before you book with a new provider, pull together whatever you can:
- Your most recent pellet insertion date
- The dose and composition of your last pellets (testosterone, estradiol, both?)
- Your most recent hormone labs (within the last 6 months is ideal)
- Any symptom notes you've kept
- Prior insertion history if you have it
If you can't access these — because the practice is closed and records transfer is slow — don't let that stop you from booking. A good provider will order fresh labs and work from where you are now.
How records transfer works in Pennsylvania
Under PA law, you have a right to your medical records regardless of what's happening with the practice. If the practice has closed, there's typically a custodian of records — often a physician executor or estate attorney — who handles releases. You can request your records by writing to the practice's last known address, or contact the PA Department of State for guidance. If you need help, your new provider's office can often assist with the request.
What to look for in a new pellet provider
Not every provider who offers pellets is set up for continuity. Ask:
- Do they take a complete pellet dosing history, or start from scratch?
- Do they source from a vetted 503A compounding pharmacy?
- Will they re-check your labs before re-inserting?
- Will they adjust the dose based on your prior response, not a standard protocol?
- Are they available between visits if you have questions about how you're feeling?
Chain practices and franchise clinics often run standardized protocols — one dose fits most. That works for some patients. It doesn't work well when your body has already been fine-tuned by someone else.
Timing your next insertion
- If your last insertion was less than 3 months ago, you likely still have active hormone — labs will tell your new provider the right re-insertion window.
- If it's been 3-5 months, you're approaching the end of your pellet's active period and should plan the next insertion soon.
- If it's been more than 5-6 months, your levels are probably already low — see someone sooner rather than later.
A thoughtful provider will use your dosing history + current labs + symptoms to decide the right moment, not a calendar date.
How AlphaCore handles pellet continuity
At AlphaCore Health, pellet patients transitioning from another practice are treated as continuity care first, new patients second. That means we work from your history, not a blank slate. We review whatever records you can bring, re-check labs, and discuss where your care was and where you want it to go.
90-day HRT packages range from $600–$1,299, depending on your individualized delivery method, dosing, and hormone combinations. Pellets and the insertion procedure are included when clinically chosen, alongside medication, evaluation, and follow-ups. Labs are billed separately by the lab. Every pharmacy we use is vetted for regulatory compliance.
Continuity care isn't about repeating what you had. It's about understanding what you had and refining it based on where you are now.
