Giveaway Rules & Terms

1. Sponsor

AlphaCore Health, located at 859 Nazareth Pike, Suite 12, Nazareth, PA 18064. Giveaways are conducted by AlphaCore Health and are not sponsored, endorsed, or administered by any third party.

2. Eligibility

• Open to legal residents of the United States who are 18 years of age or older at the time of entry.
• Employees, contractors, and immediate family members of AlphaCore Health are not eligible to win.
• Winning the giveaway does not guarantee that you will receive any treatment. Medical eligibility is determined only after an in-person or telehealth evaluation by a licensed clinician.
• Void where prohibited or restricted by law.

3. How to Enter

• Entries are collected through the official AlphaCore Health giveaway entry form, QR code, or other designated entry method.
• Only one entry per person per giveaway unless additional entry methods are explicitly offered.
• Multiple entries using different email addresses, phone numbers, or identities to circumvent the entry limit will result in disqualification.
• No purchase is necessary to enter or win.

4. Winner Selection

• Winners are selected at random from all eligible entries received during the giveaway period.
• The odds of winning depend on the total number of eligible entries received.
• Winner selection dates are announced at the start of each giveaway.

5. Prize Details

• Prizes are described in the specific giveaway announcement.
• Prizes are non-transferable and cannot be exchanged for cash or alternative products or services unless explicitly stated.
• AlphaCore Health reserves the right to substitute a prize of equal or greater value if the advertised prize becomes unavailable.

6. Winner Notification

• Winners will be notified by email or phone using the contact information provided at entry.
• Winners must respond within 7 days of notification to claim their prize.
• If a winner does not respond within the required timeframe, an alternate winner may be selected.
• Winners may be asked to provide proof of identity and eligibility before receiving a prize.

7. Privacy

Information collected during entry is used solely for administering the giveaway, contacting winners, and sending occasional AlphaCore Health communications. We do not sell personal information. For full details, please review our Policies page.

8. Prize Redemption, Medical Eligibility & Expiration

• The prize must be redeemed within 60 days of winner notification. Prizes not redeemed within 60 days will expire and an alternate winner may be selected.
• Winners must complete an evaluation with a licensed AlphaCore Health clinician to determine whether any treatment is medically appropriate. Treatment is never guaranteed and is offered only if the licensed clinician determines, in their sole clinical judgment, that the winner is a suitable candidate.
• Winners do not automatically qualify for all treatments. Available services, products, and treatment options are subject to the licensed clinician's assessment, medical history review, and applicable laws and regulations.
• If a winner is not determined to be a candidate for any treatment after evaluation, or if the prize is not redeemed within the 60-day window, the prize will be forfeited and AlphaCore Health may select an alternate winner.
• Participation in the giveaway does not guarantee any specific treatment, procedure, or outcome. All treatments are elective, educational, and supportive in nature. Individual results vary and are not guaranteed. Nothing on this page or in giveaway materials constitutes a medical diagnosis, treatment plan, or promise of outcome.
• The prize has no cash value, is non-transferable, and may not be exchanged, sold, or assigned to another person.
• Prescription medications and controlled substances are never awarded as prizes. Prizes are limited to consultations, evaluations, and non-prescription services.

9. Healthcare Compliance, Federal & Pennsylvania Law

• AlphaCore Health operates in compliance with applicable federal healthcare laws and the laws of the Commonwealth of Pennsylvania. Services are provided by an advanced practice provider authorized to evaluate, diagnose, and prescribe within the scope of Pennsylvania licensure.
• Giveaway entries and any subsequent medical evaluation are separate activities. Entering the giveaway does not establish a patient-provider relationship; a patient-provider relationship is formed only after an appropriate evaluation and agreement to care.
• All protected health information collected during any evaluation is handled in accordance with the Health Insurance Portability and Accountability Act (HIPAA) and applicable Pennsylvania privacy laws.
• Participation is not a substitute for emergency care. If you are experiencing a medical emergency, call 911 or go to the nearest emergency department.
• AlphaCore Health reserves the right to refuse service to any winner if providing the service would violate professional standards, licensing rules, or any federal, state, or local law or regulation.
• These rules shall be governed by and construed in accordance with the laws of the Commonwealth of Pennsylvania, without regard to conflict-of-law principles.

10. Limitation of Liability

AlphaCore Health is not responsible for lost, late, incomplete, invalid, or misdirected entries, technical failures, or any other cause beyond our reasonable control. Participation is at the entrant's own risk. AlphaCore Health's total liability arising from any giveaway shall not exceed the approximate retail value of the prize.

11. Disqualification

AlphaCore Health reserves the right to disqualify any participant who attempts to manipulate the entry process, violates these rules, or engages in fraudulent or abusive behavior.

12. Changes & Cancellation

AlphaCore Health reserves the right to modify, suspend, or cancel any giveaway at any time for any reason. In the event of cancellation, no prizes will be awarded.
