Day-0 Perinatal Exosomes · AlphaCore
Day-0 Perinatal Exosomes with GHK-Cu peptide booster to support the look of a healthier scalp and refreshed skin.
Exosomes are tiny membrane-bound vesicles, roughly 30–150 nanometers across, that cells release to communicate with one another. Each one carries proteins, lipids and genetic messengers such as microRNA. AlphaCore uses a cell-free, perinatal-derived exosome preparation applied topically after microneedling. It contains no living cells and is never injected.
Acellular means cell-free: no living cells, only what cells release.
Think of a stem cell as a factory. The secretome is everything the factory ships out. Exosomes are the sealed packages inside that shipment.
Living cells
Mesenchymal stem/stromal cells are living cells, typically sourced from bone marrow, fat, umbilical cord or placenta. They are the source that produces signals. Living-cell therapies are tightly regulated by the FDA, and none are approved for cosmetic use.
AlphaCore does not use stem cells.
Everything a cell releases
The full collection of what cells secrete: growth factors, cytokines, other proteins, lipids, and extracellular vesicles, including exosomes. Often sold as 'conditioned media.' Cell-free, but a broad, mixed cocktail.
One part of the secretome
A specific type of extracellular vesicle, 30–150 nm, with a protective membrane carrying proteins, lipids and RNA between cells. Cell-free, and a narrower, more specific component than the full secretome.
Bottom line: exosomes come from cells, are part of the secretome, and are not stem cells.
| Attribute | Stem Cells (MSCs) | Secretome | Exosomes |
|---|---|---|---|
| What it is | A living cell | Everything a cell releases | One type of vesicle a cell releases |
| Living cells? | Yes | No | No |
| Approximate size | ~15–30 micrometers (a whole cell) | Mixed: individual proteins plus vesicles | ~30–150 nanometers |
| How it's typically made | Collected from tissue and grown in a lab | Liquid collected from cells grown in a lab ("conditioned media") | Isolated and concentrated from that liquid |
| FDA status | One MSC product is FDA-approved, for a serious pediatric transplant complication; none for cosmetic use | No FDA-approved products | No FDA-approved products |
To picture the size difference: if one exosome were the size of a marble, a single stem cell would be roughly the height of a person — about 200 times larger.

Your blood, your baseline. PRP (platelet-rich plasma) and PRF (platelet-rich fibrin) are made from your own blood. That means what goes into the treatment reflects you at that moment: your age, sex, body weight and overall health.
Research shows the makeup of PRP varies widely from person to person. In one study, older adults had lower levels of IGF-1, a growth factor, and a less favorable balance of growth factors to inflammatory signals than younger adults. Body weight and sex also changed PRP composition.
What this can mean for you: if your system is under strain from age, ongoing inflammation or other health factors, your own platelets may bring that same baseline into the treatment. That's one reason results with PRP and PRF can be less predictable from one person to the next.
Where exosomes come from matters. Many exosome products are made from cells taken from adult bone marrow or fat. Those cells have aged along with the donor.
Early laboratory research suggests donor age may change what cells release. In one study, exosomes from older donors' bone marrow cells carried less total genetic cargo (microRNA) than those from younger donors, and behaved differently in lab tests. Another lab study found that vesicles from younger donors' cells shifted older cells toward a younger profile, while vesicles from older donors did the reverse.
Perinatal sources are collected from birth tissue at delivery, the youngest available source.
This research is early and mostly done in the lab. No head-to-head clinical studies prove one exosome source gives better cosmetic results than another.
| Attribute | PRP / PRF | Adult-derived exosomes (bone marrow, fat) | Perinatal exosomes |
|---|---|---|---|
| Source | Your own blood | Adult donor cells | Birth tissue collected at delivery |
| Depends on your age and health? | Yes | No, but on the donor's age | No |
| Living cells? | No (platelets, not cells) | No | No |
| Donor age | Your age | Adult, often older | Newborn tissue |

Sources
Most clinics offer adult-tissue exosomes derived from bone marrow or fat (adipose) tissue. These sources carry accumulated cellular stress, environmental wear, and signs of aging.
Perinatal sources come from young birth tissue collected at delivery, while adult sources come from tissue that has aged along with the donor. Microneedling and GHK-Cu prepare the skin; the exosome preparation is applied to support the skin's natural renewal. The manufacturer specifies approximately 30 billion exosome particles per vial.

Mild redness typically settles within 24–48 hours; many clients return to normal activity the next day. Individual experiences vary.
The words "stem cells," "exosomes" and "secretome" are often used loosely, and the details aren't always explained. Before any exosome or "stem cell" treatment, ask:
