AlphaCore Health is a Pennsylvania-based clinical practice. Care is delivered by Marie Perkowski, CRNP, FNP-BC · RN 21 years · NP 14 years · Functional Medicine Certified. A Certified Registered Nurse Practitioner (CRNP), a master's-level nursing provider, licensed by the Pennsylvania State Board of Nursing, practicing within a collaborative agreement with a Pennsylvania-licensed physician as required by 49 Pa. Code Ch. 21, Subch. C. Dr. Ese Akinyemi serves as medical director. The practice focuses on hormones, metabolism, energy, mental performance, and long-term health — with a plan built around your individual physiology, history, and goals rather than fixed treatment menus.

Adults 18 and older who are seeking individualized, root-cause functional medicine care. Our office is located at 859 Nazareth Pike, Suite 12, in Nazareth, Pennsylvania (inside Inspire Suites). Telehealth is available only for established patients located in Pennsylvania, and is not available for controlled medications.

No. AlphaCore Health is a cash-pay practice. We do not bill insurance. This allows us to deliver longer visits, individualized care, and access to therapies not typically covered by insurance.

Care begins with a Free Discovery Call (15 minutes, via telehealth). A Comprehensive Evaluation is $200; Follow-Up Visits are $150–$275 (20–45 min). We review the next step together before you book.

The consult is free (15 minutes, via telehealth). HRT all-inclusive packages start at $1,199, weight loss programs start at $349/mo, peptide programs start at $450/mo.

If clinically indicated, we may order targeted baseline labs. Labs are billed directly by the lab, and exact costs are discussed with you before ordering. Results typically return within 5–10 days, and we then review them together and discuss whether a treatment plan is appropriate.

No. The consultation is yours to use as you wish — including making an informed decision about whether ongoing care fits your needs.

The initial consultation is available as a standalone service. Ongoing prescriptions and therapies require an ongoing clinical relationship — one-time prescriptions and refills outside of active care are not provided.

Care is individualized based on your history, goals, and clinical review. Common areas include hormone optimization for men and women, thyroid support, weight management, metabolic health, sexual health, mental performance support, peptide-based care, and longevity-focused care. All therapies are offered only when clinically appropriate after understanding your situation, and only when prescribed by your clinician.

When prescribed by your clinician, prescriptions are sent directly to licensed pharmacy partners, who ship medications to your home or to the office for pickup, depending on the therapy.

Yes. Established patients have access to secure messaging through our patient portal for clinical questions, dose adjustments, and care coordination between visits.

Lab costs vary based on which panels are ordered, your needs, and your location. We order targeted, individualized panels and you pay the lab directly, rather than paying a flat fee that would inflate costs for everyone.

Labs are typically drawn at Quest Diagnostics or LabCorp locations. We can also coordinate at-home draws for certain panels when appropriate.

Yes. All lab results are reviewed and interpreted by your clinician as part of your care. New patients meet with the clinician specifically for a Plan Review Visit to review baseline labs together and discuss your clinical plan. Ongoing labs are reviewed as part of your care.

Both in-person visits at our Nazareth, PA office and telehealth visits are available for established patients located in Pennsylvania when clinically appropriate. Telehealth is not available for controlled medications.

Yes. All personal health information is protected through our HIPAA-compliant electronic health record (EHR) and patient portal. These are the only authorized channels for sharing protected health information. Phone calls, voicemails, and text messages are not HIPAA-secure and should not contain sensitive health information.

For clinical questions, please use the secure patient portal or messaging system. For scheduling, billing, or general inquiries, you may call our office. Do not include sensitive health information in voicemail or text messages.

Skin, hair, and facial concerns are addressed through individualized evaluation and follow-up care. Medications and procedures, when clinically appropriate, are included in your care plan pricing.
